Each guest can choose their Main Course, which is either Chicken Supreme or Eggplant Parmigiana. Please enter each attendee's full names (including your own), followed by their Main Course preference (i.e. "1. John Doe - Chicken Supreme, 2. Jane Doe - Eggplant Parm", etc.). As a reminder, you are submitting on behalf of everyone identified on the envelope you received.
